Complaint Regarding Program Action or Performance
Any program participant who has a complaint concerning a program that they participated in may file a complaint/grievance by submitting a letter or email to the Executive Director of the Tennessee Respite Coalition, Jennifer Abernathy, at jennifer.abernathy@tnrespite.org or TRC Attn: Executive Director, 2200 21st Ave S, Suite 310, Nashville, TN 37212.
All complaints will be handled in a timely manner. As a goal, TRC attempts to resolve a complaint within 30 days of receipt of the letter. All involved parties will be notified if an extension of the time becomes necessary.
Title VI Complaint
The Tennessee Respite Coalition is dedicated to providing services to family caregivers regardless of race, ethnicity, religion, creed, color, sex, age, national origin, sexual orientation, veteran status, disability, limited English proficiency, financial status or ability to pay for services.
If you feel like you have been discriminated against by TRC staff or volunteers, please use the attached form to file a complaint with the Title VI Coordinator.

Your complaint will be investigated by the Title VI Coordinator. If your complaint is against the Title VI Coordinator, it will be investigated by the Executive Committee of the Board of Directors. Investigation may include speaking to you directly and contacting witnesses you list in the complaint. Findings will be reported to you within 30 days of receipt of your complaint.
If you wish to appeal the findings of your original complaint, contact the Title VI Coordinator. Appeals for findings by the Title VI Coordinator will be appealed to the Executive Committee of the Board of Directors. Appeals of findings by the Executive Committee will be appealed to the full board. Findings of appeals will be reported to you within 30 days of the receipt of your appeal.
